Request Your Access Card

Submit the form to request your resident access card. You will not receive a confirmation of submittal, but you will receive an email when it is ready to pick up.

 

The processing time is generally 1-2 business days.

 

Pick up location is at The Amenity Center (4601 Meander Way).

 

Please ensure that you sign the Facilities Use Agreement before collecting your card.

 

Proof of residency requirements are listed below and must be provided when submitting an access card request.

 

Homeowner - Title/Deed of the home
Adult Occupant - Must have the Mosaic address listed on a state-issued ID

 

The first two (2) resident access cards for each household are complimentary. 

Any additional resident access cards will cost $25 each.
